Specimen Requirements

Specimen Type: 16 unstained and 1 H&E slide
Collection Instruction: 

1. Submit slides recut request separately to Pathology lab. Contact Pathology lab for  more details.
2. Contact NUH Clinical Chemistry Lab prior to test requisition.
3. Order test in Epic and send FoundationOne CDx & Heme Request form and FoundationOne_RSPL Consent Form to NUH Clinical Chemistry Lab.
